The National Case Management Network (NCMN) has ceased business operations as of March 31, 2014.
This membership based, non-profit organization proactively pursued its goal to promote excellence and professionalism for Case Management through education, the development of national Standards of Practice and Core Competencies and networking opportunities.
With contribution agreements from Health Canada in 2010 and 2012, NCMN published the
Canadian Standards of Practice for Case Management and the Canadian Core Competency Profile for Case Management Providers.
These documents continue to influence excellence and professionalism for the process of Case Management and the role of Case Management providers in Canada. Academic institutions, employers and government agencies are using both these documents to optimize Canada’s health workforce under the increasingly demanding and complex healthcare needs of Canadians. These documents remain available to all who are interested.
"As we all age, as we all live with chronic illness, and as we survive catastrophic illness and/or injury Case Managers will become an increasingly important role in coordinating care with a team of health care and social service providers. Across the country, Canadians, especially those with specialized, complex, and chronic health care needs, experience limited access to and gaps in health care and social service. Every episode of their care involves various individual providers and transfers between care settings. Case Management providers strive to meet the care coordination issues within and across health and social sectors. Case Management providers are collaborative leaders, outcomes managers, and multidisciplinary clinical experts with system savvy. With an unwavering commitment to quality, they are at the centre of coordinating care that is Acceptable, Accessible, Appropriate, Equitable, Efficient and Effective.” (Park, 2016)
